- Filled-out application form, which can be downloaded at www.bbpilipinas.com or picked up at the PCI Office (Uniprom Inc., Smart Araneta Coliseum, Quezon City)
- Photos: One close up and one full-body
- Birth certificate
- Philippine passport
- College or High School diploma
- Medical certificate
- Certificate of good moral character
- Parents’ consent if the interested applicant is below 18 years old
Applications are accepted on weekdays (Monday to Friday) from 10 a.m. to 5 p.m.
